Certification of diversity vendors

Q. Our firm reaching out to all new vendors to explore if they are considered diversity vendors. I believe there is a certification that proves the diversity status. We were thinking about creating a form to send with the W-9 for vendors to complete to affirm they are certified as a diversity vendor.
Does anyone have a recommendation for content of a formor a template to send with the W-9?
