For a TIN mismatch, should a new W-9 be requested?

Q. We have vendors that complete a W-9 form and they put their individual name and the dba name followed by and EIN. When we run through the TIN Match process we find that the legal name on line 1 does not always match the EIN, but if we then run it through using the DBA name with the EIN it comes back as a match. Should we send the W-9 form back to the vendor and request that they make the correction, or are we all right to make note of it?
