Paying a supplier in the absence of an invoice

Q. What is the best way to handle paying a supplier without an invoice? (Examples would be a business license renewal, a tax form, a permit fee, or a donation.) What do you do at your organization?

In lieu of an actual invoice, we utilize a payables request form. Along with the form, the requestor attaches backup documentation showing what is being purchased and the amount (i.e., receipt, email, etc.).
