Q. Should a 945 tax form be filed for mortgage interest backup withholding? If not, what is the correct document?

Can you please clarify the context of the payment? Does another entity hold the mortgage on a property of the bank and this was a mortgage interest payment to the other entity but member's bank doesn't have the EIN of the other entity?
