Q. My company received W9 with two boxes checked in the W9 3a section. The vendor checked individual/sole proprietor box and LLC box without providing tax classification specification (C = C corporation, S = S corporation, P = Partnership). Is this W9 completed correctly or should we contact the vendor and ask to correct W9 tax classification?
